Health equity drives the mission of the Pathways Community HUB Institute ® . Over the past three decades, PCHI has developed the Pathways Community HUB Institute ®  Model, dedicated to empowering communities in establishing transformative and sustainable community-based care coordination networks. Our innovative approach leverages existing community organizations and resources to provide vital support to residents who have been overlooked or fallen through the cracks of an already fragmented health and social care system.

We understand that for care coordination to be successful we need to focus on the whole person and work in partnership with multiple community stakeholders to align efforts toward our common goal of good health for all.

PCHI fulfills its mission by advancing the PCHI Model through technical assistance, training, certification, and research. Your support can make a significant impact by contributing to our efforts.
