PCHI highlighted in policy paper on sustainable funding for community health workers


Partners In Health United States (PIH-US) – a nationally leading non-profit organization focused on making healthcare accessible for everyone – recently released a policy paper providing recommendations to state policy makers on how to establish more sustainable funding for community health workers (CHWs). The PCHI® Model is included in this report as an “example of a nationally replicated CHW model that ties payments to quality measures to incentivize the provision of whole-person, coordinated care.”


CHWs play an essential role in better addressing social determinants of health and health inequities. In fact, CHWs are central to the PCHI Model. As trusted members of their communities, CHWs work with and connect residents to needed health and social services.

PCHI is honored to have a role in PIH-US’s latest report and in helping to inform and educate state policymakers about the CHW workforce.
