Four Pathways Community HUBs Earn Certification and Re-Certification to Close 2024

PCHI® is excited to announce updated certification status of two Pathways Community HUBs (PCHs) and the re-certification of two others. These PCHs are making a difference in advancing health equity across diverse communities.

 

Updated Certification Status Pathways Community HUBs:

 

Re-Certified Pathways Community HUBs:

  • Community Care HUB (Johnstown, Pennsylvania) and Dayton Regional PCH (Dayton, Ohio) have earned full recertification for the next three years, effective from November 2024 to November 2027.

 

The PCHI Certification Program, established in 2013, ensures that PCHs adhere to the PCHI® Model—a community-based care coordination framework leveraging a value-based payment system. The Model brings together community organizations to address modifiable risk factors—medical, social, and behavioral—by connecting individuals to services, tracking outcomes, and aligning payments with measurable results.

 

Congratulations to these PCHs for their dedication and impact. They are making a difference in their communities, linking people to the support and resources they need to live healthier, fuller lives.
