The Pathways Community HUB Institute provides:

• Technical assistance and consulting services to align with the PCHI® Model. PCHI advises communities, agencies, health systems, payers, government and other types of organizations that aspire to achieve the benefits of the PCHI Model.

• Certification in the PCHI Model for Pathways Community HUBs, Pathways Agencies and Technology Vendors.

• Education and Training on the Pathways Community HUB Institute Model for Pathways Community HUBs, Pathways Agencies, and Community Health Workers.

• A PCHI Action Network sponsored by PCHI to further community-based care coordination with a focus on the PCHI Model. Educational and information sharing virtual meetings are held quarterly.
